Chapter reporting Form for convention- send this to the District Governor.

AHEPAN of the Year Application send this to the District Governor.

(National Form is to be used as All District 20 for current year recipients will be submitted to national. Please fill out form as complete as possible the submit to current District Govenor by May 1st of each year)

Chapter Election Results for the District - send this to the District Secretary. You may also use the National Chapter Elections Results form instead.

Chapter Delegate Reporting Form for the District Convention- send this to the District Secretary. El Camino Ad Form- send this to the District News Editor.

Per Capita Reporting (For Any Year) - send this to the District Secretary. The Secretary will record the transaction and forward the monies on to the District Treasurer.
